June 12, 2021 - Get Well Now with Dr Doug Pucci - getwell-now.com - In mulling over an email from a complete stranger, Dr Pucci ponders mental decline & what’s at the root of “oopsey-daisy” thinking. Why do we get caught up, trapped in our own mental isolation? How is this advanced by the pandemic? What causes false beliefs, wrongful accusations and false assumptions. Moreover, how does this further hinder our own attempts to heal?

A few weeks ago we released the emerging concept in our newsletter that our brain is primarily an “immune” organ. That it is not just the center of thought, or the computer inside our heads —that, indeed, it is primarily an immune organ that is quickly inflamed and subject to all kinds of blood sugar (and other) fluctuations. About 90% of the cellular construct of the brain are immune cells, called glia, and (only) about 10% are neurons. Neurons are the signaling cells that constitute thought and movement, and the glia are those that scavenge, do the clean up and keep the brain tidy and balanced.

When we think about the brain, and the components that lead to “thought” and “action”, we see that the brain divides into two hemispheres. Structurally, there is a left and a right hemisphere, and functionally, especially regarding “executive” function of the frontal lobe, or higher brain, there is also a “stop” and a “go” aspect. In fact, it’s fair to say that the major function of the brain’s executive function is inhibitory; meaning, if the primordial instinct is to “go”, or to “give flight”, it is the higher brain that says “Stop, let’s reflect on this”.

One of the signs that a person’s mental health is suffering is that they slowly start to “lose the brakes”. They lose the ability for inhibition. There are motor aspects of brain decline that result in movement disorders, such as tremors, and also frontal lobe aspects related to cognition. There is a lack of “future” casting, or seeing the consequences of an action or decision. Another word for this is “blindspot”.

When histamine is able to cross the blood-brain barrier it can create the out-of-control “brain on fire” reality. Long term, like a wildfire that burns out of control, prolonged histamine exposure leads to mental decline. One of the ways I talk about this in my clinic is with the analogy of a campfire. It might release a few embers to start. All it takes is a few gusts of wind and some dry kindling to become a forest fire. At this point, it’s all a firefighting unit can do to simply “contain” the blaze. It is too hot to go near and even “put out”. In a patient, this is full-fledged dementia or irreversible brain decline, where the goal is to contain it from worsening, or spreading.
