In this Orthopaedic Surgery video lecture, Dr. Hammad Haider goes into detail about Tuberculosis (TB). It is essentially described as an inflammation, occurring due to the Mycobacterium tuberculosis infection.

Pulmonary tuberculosis (TB) is seen in almost 60-70 % patients, but extrapulmonary manifestations of TB also exist. These are common in immunocompromised patients, but the joint (osteoarticular) infection is quite rare.

In adults, osteoarticular tuberculosis (TB) shows a preponderance to the spine (40%), then the hip (25%), and finally the knee joint (8%).

✔️ Knee Tuberculosis Pathology
The progression and consequent events occurring in the knee are explained, as the synovium reacts to the Mycobacterium infection.

✔️ Knee Tuberculosis Clinical Features
As a result of the infection in the tuberculosis of the knee, the patients display multiple symptoms. These should be kept in mind while moving forward with the proper diagnosis, which may include swelling and pain in the joint.

✔️ Knee Tuberculosis Diagnosis
The diagnostic procedure of knee tuberculosis starts with laboratory tests, it displays elevated ESR and a positive Mantoux test. Followed by characteristic appearance via radiological studies.

✔️ Knee Tuberculosis Differential Diagnosis
Differentials must be kept in mind when diagnosing knee tuberculosis. Along with physical examination other includes the naked eye evaluation of cross section of the knee and biopsy to rule out monoarticular affections, rheumatoid arthritis, synovial chondromatoses, hemarthrosis, synovioma, foreign body granuloma, and dysenteric arthritis.

✔️ Knee Tuberculosis Non-Surgical Treatment
After the conservative treatment involving the pharmacotherapy of tuberculosis, the diseases management, includes anti-tuberculosis chemotherapy for 12-18 months. Also, the use of a bed splint, which supports the knee and aids recovery.

✔️ Knee Tuberculosis Surgical Treatment
The surgical treatment of knee tuberculosis depends upon the site and progression of the disease. At the synovial stage, arthrotomy and synovectomy are performed. Arthrodesis and arthroplasty could also be options.

The post-operative treatment includes triple drug therapy, traction, intermittent active and assisted exercises, and suitable brace ambulation.
