Seguimos presentando pacientes a los que llevamos siguiendo hace mucho tiempo, para aumentar nuestro conocimiento de la “evolución a largo plazo” de nuestros trasplantes. Tras 17 años de experiencia en técnica FUE (2003-2020) creemos importante tener ejemplos de este tipo de casos para aumentar nuestra información sobre estrategias a largo plazo, cuando nos planteamos por primera vez realizarnos un trasplante. Recordarles que hace 10-15 años las estrategias, número de grafts empleados y resultados no eran los mismos que se consiguen actualmente.
Este paciente fue visto por primera vez en 2010 a la edad de 32 años. Llevaba 3 años con finasteride, aunque empezó a notar los efectos de la alopecia con 26 años. Antecedentes familiares de alopecia avanzada y un patrón difuso NW IV, aunque con pelo mojado presenta una difusa de toda la zona superior (DPA NW V). Donante corta con pelo de 55 a 58 micras. Su primer objetivo fue reparar la zona frontal.

En la primera operación se extrajeron 1601 grafts a 2,16 pelos/grafts (3463 pelos), con punch de 0.8 mm.
Desglose: I- 235; II-900; III-436; IV-30.
Por la poca cantidad de unidades empleadas para la zona tratada, el resultado final no logró la cobertura planeada por lo que a los 2 años, nos encontramos otra vez en quirófano para una nueva cirugía. Se vuelven a extraer 1601 FUE Grafts a 2,27 pelos (3640 pelos), con punch de 0.8 mm. Se baja ligeramente la línea frontal y se refuerza coronilla con algunos grafts.
Desglose: I-181; II-848; III-526; IV-45; V-1
Más contentos con el resultado obtenido, el paciente vuelve dos años después (2015) para restaurar la zona media y los picos temporales. Se extraen 1101 grafts a 2,44 pelos/graft (2686 pelos) con punch de 0.8 mm serrado.
Desglose: I-85; II-515; III-433; IV-68.

Último retoque 3 años después (2018) para bajar ligeramente la línea frontal. Estadísticas:

· Temporal: 55 micras de pelo; 72 UF/cm2; 147 pelos/cm2; 2.04 pelos/UF; CV de 8,09

· Parietal: 57 micras; 73 UF/cm2; 146 pelos/cm2; 2,00 pelos/UF; CV de 8,32
· Occipital: 58 micras; 70 UF/cm2; 156 pelos/cm2; 2.23 pelos/UF; CV de 9.05

TDA (total donor area)= 236 cm2; TDC (Total donor capacity)= 6,152; TDHC (total donor hair capacity)= 12,858

Se extraen 351 grafts a 2,20 pelos/graft (771 pelos) con punch de 0.8 mm. Desglose: I-89; II-125; III-116; IV-21.

El video muestra el resultado final a los 10 meses de esta última cirugía con TDC residual suficiente para el futuro.

We continue to introduce patients whom we have been following for a long time, to increase our knowledge of the “long-term evolution” of our hair transplants. After 17 years of experience in the FUE technique (2003-2020), we believe it is important to have examples of this type of case to increase our information on long-term strategies, when we first considered having a hair transplant. We want to remind that 10-15 years ago, the strategies, number of grafts used and results were not the same as those currently achieved.
This patient was first seen in 2010 at the age of 32. He had been on finasteride for 3 years, although he began to notice the effects of alopecia when he was 26 years old. A family history of advanced alopecia and a diffuse NW IV pattern, although with wet hair, he has a diffuse alopecia of the whole upper zone (DPA NW V). Hair caliber from 55 to 58 microns. His first objective was to repair the frontal area.
In his first procedure.
1601 grafts were extracted at 2.16 hairs/grafts (3463 hairs), with a 0.8 mm punch.

Due to the small number of grafts used for the treated area, the final result achieved was not de ideal, so after 2 years, we were again in the operating room for a new procedure. 1601 FUE Grafts were extracted at 2.27 hairs (3640 hairs), with a 0.8 mm punch. The front line was lowered slightly and the crown was reinforced with some grafts.
Breakdown: I-181; II-848; III-526; IV-45; V-1.
More happy with the result obtained, the patient returns two years later (2015) to restore the middle zone and the temporal peaks. 1101 grafts were extracted at 2.44 hairs/graft (2686 hairs) with a serrated 0.8 mm punch. Breakdown: I-85; II-515; III-433; IV-68.
Last touch-up 3 years later (2018) to slightly lower the frontal hair line again. Statistics:
· Temporal: 55 µm of hair; 72 FU/cm2; 147 hairs/cm2; 2.04 hairs/FU; CV of 8.09

· Parietal: 57 µm; 73 FU/cm2; 146 hairs/cm2; 2.00 hairs/FU; CV of 8.32

· Occipital: 58 µm; 70 FU/cm2; 156 hairs/cm2; 2.23 hairs/FU; CV of 9.05
